This is an online e log book to discuss our patient de-identified health data shared after taking his / her / guardians signed informed consent. Here we discuss our individual patients problems through series of inputs from available global online community of experts with an aim to solve those patients clinical problem with collective current best evident based input. A 55 year old male patient presented to the opd on 8-11-21 with chief complaints of bilateral pedal edema since 10 days ,shortness of breath since 4 days , facial puffiness and decreased appetite since 2 days . HISTORY OF PRESENT ILLNESS Patient was apparently asymptomatic 3 years back and then he developed right sided hemiparesis and was diagnosed with CVA and was on antiplatelets. 2 years back he was also diagnosed with diabetes and hypertension.
